Laparoscopic Surgery in Esophageal Achalasia: 3 Cases Report

Abstract
Esophageal achalasia is an idiopathic motor alteration of the smooth esophageal muscle, and this is characterized by absent post deglutition relaxation of the lower esophageal sphincter (LES). Achalasia is the best known and most common disorder among the primary and specific esophageal motility alterations, and it has an incidence of 0.5- 1 case per 100,000 yearly with an age range of 30 to 50 years in USA. We describe the three achalasia cases with symptomatic gastro-esophageal reflux that was proven by radiology or endoscopic 24 hours pH monitoring. They initially complained of dysphagia that accompanyed the reflux, which became then severe and resulted in esophageal stasis. Balloon dilatation was performed successfully, yet the symptoms recurred after a short period. This was not improved with repeated balloon dilatation and so the patients were transferred for surgical intervention. All were treated with laparoscopic esophageal myotomy with Nissen¡¯s fundoplication. After 6 months to 2 years of follow up, there have been no recurrences.
